APG’s 2024 Case Studies in Excellence is now available.
The case studies in this 2024 volume highlight 12 APG member organizations’ initiatives that advance high-value care in five key areas: Addressing health disparities and social determinants of health; timely access to mental health supports and services; optimizing care access and spend; pharmacist integration in primary care; and technology solutions to drive high-value care.
About APG Case Studies in Excellence
Since 2011, APG has published a periodic compendium of deep-dive case studies that demonstrate innovative, high-quality health care initiatives designed and implemented by APG member organizations. Each annual edition highlights the exceptional value-based care our members provide to their patients and aims to share best practices across the APG membership and with other organizations. For policymakers, purchases/payors, and other health care stakeholders, Case Studies in Excellence also helps tell the story of what “value” looks like in frontline clinics with patient-centered, high-quality care models.
Previous issues of Case Studies in Excellence have featured excellent examples of inpatient-ambulatory transitions coordination, preventive and chronic care innovations, end-of-life care navigation support, complex care coordination, among others.
