As the 65+ population rapidly expands—expected to reach 80 million by 2040—health systems face mounting challenges in delivering high-quality, cost-effective care while navigating workforce shortages, provider burnout, and shifting reimbursement models. This session will explore practical, person-centered approaches to care that enhance patient engagement, reduce provider burden, and support long-term health system viability.

Attendees will gain insights into

  • Understanding how older adults view value and loyalty in other service sectors
  • Addressing rising costs related to physical inactivity and falls
  • Improving quality measures
  • Addressing provider burnout with proactive, community-based solutions
  • Implementing evidence-based programs that promote physical activity and fall prevention—without increasing clinic visit demand
  • Enhancing patient satisfaction to help retain Medicare beneficiaries

Led by experts in geriatrics, health system leadership, population health, and patient engagement, this session will provide actionable solutions for optimizing internal and external resources. Learn how to create meaningful, patient-driven care models that support “What Matters” most to older adults, ensuring both immediate impact and sustainable success in a value-based care environment. This approach addresses physical activity and fall risk reduction for older adults as a framework for implementing evidence-based and patient-centric programs (at home) that do not require valuable provider clinic visit time.
