Thank you for registering for our webinar on CMMI’s new model: Making Care Primary. The RFA (request for applications) is now open and applications must be submitted by November 30, 2023. This model will operate for 10 ½ years and will be initially based in 8 states (Colorado, Massachusetts, Minnesota, New Mexico, New Jersey, Upstate New York, North Carolina, and Washington).
Sarah Fogler, CMMI Acting Director Patient Care Model Group and Melissa Cohen, Coral Health Advisors (Former Group Director of the Policy and Programs Group at CMMI) discussed the model goals, elements, application process, and key elements to assess if the model is right for your organization.
