Washington, DC – America’s Physician Groups (APG) congratulates its 13 member and sponsor organizations that are among the 110 groups provisionally accepted to participate in the Accountable Care Organization Realizing Access, Equity and Community Health (ACO REACH) Model for performance year 2023. Two of the groups, agilon health and Iora/One Medical, were previously in earlier phases of the program. All will now join the 15 other APG member organizations who also have been participating in the program.
According to the Center for Medicare and Medicaid Innovation (CMMI), fewer than half of applicants to this new ACO REACH cohort were approved for participation. CMMI has determined that this is the last group of new participants that will be admitted to the ACO REACH program. In its List of Provisionally Accepted Applicants and IP3 Participants notice on August 15, CMMI points out that all 110 provisionally accepted groups must meet specific requirements for final approval to begin participating in ACO REACH beginning January 1, 2023. The ACO REACH Model runs through December 2026.

CMMI requires that all ACO REACH participants develop and implement a robust health equity plan to address and measurably reduce health disparities in their communities. APG is committed to working with its members and CMMI in developing optimal health equity strategies to improve health care access and outcomes for Medicare beneficiaries in underserved communities.
ACO REACH, formerly named the Global and Professional Direct Contracting Model (GPDC), is the first accountable care model in Medicare to directly address issues related to health equity and access. ACO REACH participants will be held financially accountable for the total health and cost of care for the fee-for-service Medicare patients who are attributed to them either through claims or voluntary alignment. The model provides for a holistic, accountable approach to treating Medicare beneficiaries, improving care for those with chronic conditions, addressing their social needs related to health and health care, and achieving greater health equity in the process.
