This program is continuously improving and garnering broader support across the state, as beneficiaries begin to experience the positive benefit of care coordination across the spectrum of their medical, behavioral and social needs. Over 160 physician groups across California remain committed to expanding access to coordinated patient care delivery in our healthcare system.”
